Pre-Release/Transition
Case Managers work with incarcerated
youth prior to release, preparing needs assessments and
establishing a plan of action and services needed upon release
in an effort to prepare for a smooth transition to life
on the outside.
We have Case Managers who work with incarcerated
youth prior to release, preparing needs assessments and
establishing a plan of action and services needed upon release
in an effort to prepare for a smooth transition. These clients
face particular challenges before they are ready to begin
looking for work, and often need more intensive hands-on
attention to navigate the transition from the highly-regulated
life in prison to life on the outside. Transition needs
might include successfully enrolling in school, setting
up regular meetings with parole/probation officer, finding
housing, obtaining proper identification and attending assigned
classes (anger management, drug treatment, etc.). Our pre-release
staff is trained in working with this population, and is
committed to assisting these most-challenged young people.
